Please
find below the steps to upload all the required documents on App.
Step
1: Download the "Avon Flex" app from your Play store/App Store
Step
2: Click the Login Option at the bottom right corner of your screen (Note do
not use the Sign-Up option but rather the Log In option)
Step
3: Click on the Forgot Password option, input your Username/Member Number which
it is your Numeric Avon ID number (xxxxxx), then submit.
Step
4: A default token/password would be sent to your registered email address. Use
the information gotten to Login on the Avon Flex application, then change your
password to your preferred option to login successfully.
Kindly
note that the reset token will be sent to the registered email on your HMO
profile (
xxxxxxxxxxx@yahoo.com).
Step
5: Navigate to the Action Page > Click the self-service option > Click
Request for a refund > Click continue after reading the guidelines.
Step
6: Proceed to fill in your details in the required fields.
Step
7: Upload in their respective allocations:
Detailed
Medical Report
Receipts
(All receipts should be scanned together as one document)
Itemized
Invoice that shows breakdown and price for each services done (All Invoices
should be scanned together as one document)
